We are unable to treat for both mental health and physical rehabilitation in one visit. Please prioritise the area of treatment needed (either physical rehabilitation or mental health) and fill in the application form accordingly. You may of course apply for additional treatment another time.

Our physical rehabilitation patients have access to health and wellbeing classes, and our mental health patients are very welcome to use the swimming pool and gym.

You may be asked to provide additional documentation during the application process and your application will be unsuccessful if this is not provided.

Once registered, you will be able to login and complete your application. Applications auto-save section by section, should you need to pause the process.

Note: allow yourself at least 40 minutes to complete your application.

You will need;

• Your Patient Summary from your doctor. Please use our letter (PDF) to help make the request. You can upload this. We cannot process your application without it.
• Your doctor’s surgery contact details, including name, address, email address and your doctor’s name.
• Your NHS number. You can find details on the NHS website.
• A clear portrait photo of yourself.
• Police service contact details: including name, phone number and email address of your Force Welfare Officer or Federation Representative or Occupational Health Lead or your Line Manager.
• Details of who to contact in an emergency: including name, phone number and email address.
• Dates you are unavailable for treatment, like pre-booked holiday.
